This agreement is by and between L.I.T. Dance Fitness, LLC (“LIT Dance”) and the participant.

In signing below I hereby agree to the following:

1. I am participating in fitness classes and during which, I will receive information and instruction about dance and/or fitness. I recognize that dance and/ or fitness classes require physical exertion that may be strenuous and may cause physical injury, and I am fully aware of the risks and hazards involved, and I fully and voluntarily accept such risks.

2. I understand that it is my responsibility to consult with a physician prior to and regarding my participation in the dance and/or fitness classes. I represent and warrant that I am physically fit and I have no medical condition that would prevent my full and safe participation in the dance and/or fitness classes.

3. In consideration of being permitted to participate in dance and/or fitness classes, I agree to assume full responsibility for any risks, injuries or damages, known or unknown, which I might incur as a result of participating in the program.

4. In further consideration of being permitted to participate in dance and/or fitness classes, I knowingly, voluntarily and expressly waive any claim I may have against LIT Dance and its employees, it’s dance and/or fitness instructors, and it’s agents for damages, and injury, including death that I may sustain as a result of participating in dance and/or fitness classes.

5. I, my heirs or legal representatives forever release, waive, discharge and covenant not to sue LIT Dance, and it’s employees, it’s dance and/or fitness instructors, and it’s agents for any injury or death caused by my voluntary participation in the dance fitness classes.

6. I agree that LIT Dance is in no way responsible for the safekeeping of my personal belongings while I attend class.

7. I understand that LIT Dance may photograph, videotape, film, or record my participation in the activities or use of this facility and hereby agree to consent thereto. I am also aware that LIT Dance may use, publish, transmit, transfer or reproduce such recordings for promotional, marketing, educational, or instructional purposes, or other purpose related to LIT Dance and hereby consents to such use. LIT Dance shall retain rights to all recordings.

I have read the above release and waiver of liability and fully understand its contents. I voluntarily agree to the terms and conditions stated above. This agreement remains in effect for as long as I participate as a client at LIT Dance.
